                 MEMORANDUM OF UNDERSTANDING FOR CLIENTS SEEKING 

                                VETERINARY SERVICES FROM PUBLIC VET

Public Vet policies are in accordance with requirements of State, National and International Veterinary Associations. All Public Vet clients are required to have a Veterinarian-Client-Patient-Relationship (VCPR) for each animal prior to admission to the clinic.  The client is assumed to be the owner.

If you are not the owner of the animal, the information below will define your responsibility as a Public Vet client.

We perform surgical procedures on patients and are responsible for providing post-operative support.  

As a Public Vet client seeking veterinary services for an animal that you do not own, you agree to the following:

1. You acknowledge that you are not the owner of the animal and that you are acting as an agent with owner consent, a rescue housing the patient, a foster for an organization, a shelter housing the patient in a facility, a caretaker, or a good samaritan surrendering the animal to Public Vet.
2. You understand that Public Vet policies require a Veterinarian-Client-Patient-Relationship (VCPR) for each animal prior to admission to the clinic. Therefore, you agree to provide the necessary information about the owner and the patient at the clinic and online.
3. You agree to provide the physical location of the patient after surgery as well as the primary post-operative caregiver’s contact number for the recovery period. You also agree to maintain direct lines of communication with caregivers after surgery to ensure safe recoveries.
4. You agree to provide full disclosure of the animal’s history, the animal’s living situation, and the caregiver for the animal to be most effective in promoting health.
5. You acknowledge that Public Vet is not a regulatory or enforcement agency and that its goal is to assist communities in creating healthy animal companionships. You understand that Public Vet networks with other animal welfare organizations to meet the need for animal welfare.
6. You agree to provide owner consent to attend the clinic (if there is an owner) and acknowledge that the owner is assumed to be the client.

By signing your paperwork at check-in, you acknowledge that you have read and understand the terms of this Memorandum of Understanding and agree to comply with all Public Vet policies and procedures.

This is an agreement between Public Vet, animal welfare organizations or individuals that bring animals to the clinic.
